The failure was more of inadequate police arrangement and prior coordination between the several agencies involved -- the DC Metropolitan Police, the National Guards, the FBI, the New York and Maryland State Guards. It would be fair to conclude that the US administration was not proactive in anticipatory preparation since the rally behind the White House , and the ensuing mob, was of Trump supporters.
